Untuk sebuah Proyek Pembangunan Konstruksi atau pemasangan pipa dan mesin, diperlukannya Polis Contractors’ All Risks Insurance atau Erection All Risks Insurance, untuk memberikan proteksi kepada proyek yang sedang dikerjakan.

 

Untuk membuat polis tersebut, diperlukan beberapa dokumen untuk penerbitannya untuk di review oleh para Underwriter (Perusahaan Asuransi). Beberapa informasi yang diperlukan adalah sbb :

4. NAME OF PRINCIPAL
5. REQUIRED SUMS INSURED :

(a)  contract work;

Temporary works

(b)  third party liability

(c)   Existing / surrounding property of the project area

4. PERIOD :

(a)   construction & starting date

(b)   testing / commissioning

(c)   maintenance

5. FULL DESCRIPTION OF PROJECT

(please answer as fully as possible so allowing insurers to form a clear and accurate understanding of work to be undertaken)

6. SITE INFORMATION :

(a)   particulars of soil conditions

(b)   altitude above sea level and proximity to water

     7. WILL THERE BE ANY WET WORKS AND/OR OFFSHORE WORKS TO BE DONE?

8. PROJECT COST :

(a)   estimated full contract value

(b)   breakdown of contract value (or on major items of overall project)

9. REQUIRED COPIES OF THE FOLLOWING :

(a)   site plot plant /  lay out of the project

(b)   works progress chart / bar chart / time schedule

(c)   full contract agreement conditions

(d)   scope of works
